Backflow Works Service Agreement

Annual service costs $199 per enrolled backflow assembly. It renews each year unless canceled. We'll notify you before your renewal date. Testing, filing, ordinary repairs, and related retesting are included.
1. What is included
For each enrolled backflow assembly, your annual service includes:
- Annual backflow testing
- Required filing or submission of test results
- Repairs required due to ordinary mechanical failure
- Retesting when required following a covered repair
A repair restores your existing backflow assembly to proper working condition. A replacement means installing a new assembly because the existing one can no longer be repaired. Repairs are included in your annual service. Replacement is not—see Section 3.
Testing and repairs are performed by qualified professionals.
2. Annual price
Service costs $199 per enrolled backflow assembly per year.
Your total annual price is based on the number of assemblies you enroll.
3. What is not included
The following are not included in the annual service price:
- Damage
- Theft
- Replacement of the backflow assembly
If an assembly requires replacement or other work that is not covered, we will notify you and obtain your approval before performing work that would result in an additional charge.
4. Scheduling and access
You agree to provide accurate contact and property information and reasonable access to each enrolled assembly so we can perform testing, repairs, and retesting.
If service cannot be completed because access is unavailable, we may reschedule. If repeated access problems result in additional costs, a rescheduling fee may apply. We will disclose the fee before charging it.
5. Assembly information
You do not need to know every assembly detail when you enroll. We may confirm or update information later, including the manufacturer, model, serial number, size, type, condition, and location.
If we identify an additional assembly that is not enrolled, we will notify you. It will not be covered unless you choose to enroll it and pay the applicable annual price.
6. Annual renewal and price changes
Your service renews automatically each year at the annual price then in effect for each enrolled assembly unless you cancel before the renewal date.
We will notify you before your renewal date.
Backflow Works may change the annual price for future renewal periods. We will notify you before charging the new price, and you may cancel before the renewal date.
You authorize Backflow Works to charge the payment method on file for each annual renewal.
7. Cancellation and refunds
You may cancel future renewal at any time. To avoid the next annual charge, cancel before the renewal date.
You may request a refund before the first service visit for the applicable annual period. Once that visit occurs, the annual fee for that period is non-refundable.
Cancellation prevents future renewal. Coverage for the current paid annual period continues through the end of that period.
8. Cancellation by Backflow Works
Backflow Works may cancel your service if we cannot reasonably provide it, if safe or reasonable access is not provided, or if you do not meet your payment or other responsibilities under this Agreement.
We will notify you if we cancel your service. If cancellation occurs before your first service visit for the applicable annual period, we will refund the annual fee for that period.
9. Responsibility for service
Backflow Works performs all work with reasonable care.
Backflow Works is not responsible for pre-existing conditions, damage unrelated to our work, or problems with plumbing or property outside the enrolled assembly.
10. Customer authorization
You authorize Backflow Works and its qualified service providers to access the property at agreed times and to test, file results for, repair, and retest the enrolled assemblies.
You confirm that you are the property owner or are otherwise authorized to approve this service and provide access to the enrolled assemblies.
11. Payment and start of service
Service begins after successful payment. If payment is unsuccessful, Backflow Works may delay scheduling or suspend service until payment is completed.
You are responsible for keeping your billing and contact information current.
12. Acceptance
By completing your purchase, you acknowledge that you have read and agree to this Service Agreement, including its annual renewal and cancellation terms.
